How to choose and use a brain sheet

Start with the sheet built for your unit. An ICU sheet keeps vent settings, drips and hemodynamics in view. A Med-Surg sheet organizes a multi-patient assignment of meds, labs and assessments. A Labor and Delivery sheet follows contractions, fetal heart tones and cervical changes.

Print it before report and fill in each patient as you take handoff. Keep it with you all shift and update it as new orders, results and vitals come in, so your end-of-shift report is mostly written by the time you give it.

A paper sheet holds patient information, so follow your facility's policy: keep it on you, never leave it at the nurses' station, and put it in a confidential shred bin at the end of your shift.
